Internal vs. Rigid External Distraction Device for the Maxillary Hypoplasia of Cleft Patients

چکیده

McCarthy에 의해 하악골의 길이 신장을 위한 골신장술 (Distraction Osteogenesis)이 사용된 이후, 반안면왜소증 (hemifacial microsomia)등 하악골뿐 아니라, 상악골을 포 함한 중안면골의 전진, 임플란트 식립을 위한 치조골 증강 술, transport distraction을 이용한 악골의 재건 등 다양 한 방면에서 골신장술이 응용되어 왔다. 구순구개열과 동반된 상악골의 열성장은 구순구개열 환자 의 22�27%에서 수술을 통한 상악골의 전방이동이 필요하 다고 보고되어 왔다. 악교정수술을 통한 상악골의 전 진술이 주로 사용되어 왔으나, 상악의 반흔으로 인해 6mm 이상의 상악골 전진은 한계를 가지는 것으로 여겨 진다. 이러한 한계점으로 인해 전치부의 반대교합을 해소할 목적 으로 정상 성장을 보이는 하악골에서도 후퇴술이 불가피하 게 시행되며, 이 과정에서 하악의 후퇴를 보상하기 위한 이 부전진술이 필요한 경우도 발생하게 된다. 최근 골신장술은 구순구개열 환자의 상악골 열성장을 위 한 우선 고려되는 치료법으로 자리잡고 있다. 연조직을 포함한 상악골의 점진적인 전진술은 충분한 상악골의 전진 을 가능하게 하여 악교정수술의 한계를 극복할 수 있는 것 으로 선호되게 되었다. 대한악안면성형재건외과학회지: Vol. 27, No. 4, 2005
